Heather Crowell, LCMHC, MLADC, LCS

I’ve been working in the Mental Health field since 2007. I’m a Licensed Clinical Mental Health Counselor and a Master Licensed Alcohol and Drug Counselor.  Throughout my career I have worked with a variety of populations. My specialties include co-occurring disorders, perinatal populations, women's health, and Borderline Personality Disorder.

Substance Use Disorder Evaluations

Substance Use Disorder Evaluations are often required as part of legal requirements set by the court system. A LADC Evaluation is often used to determine if someone has a Substance Use Disorder and to make treatment recommendations. 

LADC Evaluations involve a combination of a clinical interview and assessment. They are often completed in 1-3 sessions. 
There are a number of reasons that someone
would need this type of evaluation including: -Driving While Intoxicated/Under the Influence-Possession of a substance -Assault where substances are involved
These types of evaluations are not typically covered under insurance. The cost of this evaluation is $375.  Please contact us for more information on LADC Evaluations.

No Surprises Act-
Right to Receive a Good Faith Estimate of Expected Charges


Under the No Surprises Act, you have a right to receive a "Good Faith Estimate"explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as medical test, prescription drugs, equipment, and hospital fees.

​

Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item.  You can also ask your health care provider, and any provider you choose for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate for your records.
